We remind you that this form must be compulsorily completed online and in English.
Specify the department or administrative unit to which the designated coordinator belongs.
Provide the full legal name of the person designated as coordinator.
State the current position of the coordinator within your institution’s organisational structure.
This should be the coordinator’s primary institutional email for all official communication.
Provide a direct contact number to facilitate communication when necessary.
This information helps confirm that the appointed person is duly authorised to manage programme-related tasks.
Provide a brief description of the main tasks and responsibilities of the coordinator. Include oversight of the mobility offers, communication with the departments, and liaison with the CGU Executive Secretariat.
Indicate the start and end date for the coordinator’s designation. This will define the period during which the coordinator is responsible for the management and communication of mobility offers.
If no specific term of appointment has been established, indicate that the appointment will remain active until a new appointment is made.
Provide the full name of an alternate contact person, in case the designated coordinator is unavailable. This person will only be contacted if necessary.
Provide the email address of the alternate contact. Make sure it is active and regularly checked to ensure smooth communication.
Enter the full name of the person with authority to approve the designation of the coordinator. Typically this is a department head, HR director, or equivalent.
